KVKK Explicit Consent Statement

Within the scope of the Privacy Notice on the Processing of Personal Data presented to me, and pursuant to Law No. 6698 on the Protection of Personal Data, I give my explicit consent to the processing of my personal data — identity, contact, user transaction, location, IP and cookie data, together with the special categories of personal data processed within the scope of health services — for the purposes of creating a mobile application membership, sending SMS verification codes and providing access to my account.

I accept that my personal data may be processed in an integrated manner with the call centre, ambulance service and medical intervention processes in emergencies; processed by physicians and healthcare staff for the purposes of medical diagnosis, treatment and care during the provision of health services; and shared, in accordance with the legislation, with technical support, call centre, security infrastructure, advertising and analytics service providers.

I consent to my usage habits being measured anonymously through the cookies, advertising tags and similar technologies used in the application, and to the company sending commercial electronic messages for informational, announcement and promotional purposes. I have been informed that I may withdraw this consent at any time and exercise my right to refuse.

I permit my visual and audio records, such as photographs and/or video, to be used in the company’s informational and promotional activities concerning health services only where my explicit consent has been given.

I am aware that my data may be shared domestically and, where required by legislation, with public institutions; that the company shall not be liable in the event of unlawful interference by third parties; and that I hold all the rights set out in Article 11 of the KVKK.

I give my explicit consent of my own free will and accept, declare and undertake the matters set out above.
